Implants Can't Decay, but They Still Need Care

A common misunderstanding is that once an implant has healed, the work is done. It's true that a titanium or zirconia implant cannot get a cavity. But the tissues around it, the gum collar and the supporting bone, respond to plaque bacteria much as the tissues around natural teeth do. Left unchecked, inflammation around an implant can progress to bone loss and, ultimately, implant failure.

Peri-implant disease is largely preventable with consistent home care and regular professional maintenance. With proper care, implants can last many years; individual results vary. Daily Hygiene Around an Implant

The daily routine for an implant is familiar but deserves particular attention at the gumline, where the crown emerges through the gum. This is the zone where plaque accumulates and disease begins:

  • Brush twice daily with a soft-bristled manual or electric toothbrush, angling bristles toward the gumline around the implant crown. Low-abrasive toothpaste is preferred, as heavily abrasive pastes can scratch restoration surfaces over time.
  • Clean between teeth every day. The spaces beside an implant crown often differ slightly in shape from natural tooth contacts, so your hygienist will show you which tool fits your specific restoration.
  • Pay attention under bridges and bars. Implant bridges and full-arch prostheses have areas beneath the connecting framework that a regular brush cannot reach; these need dedicated tools daily.
  • Antimicrobial rinses may be recommended for short periods, for example after surgery or during treatment of gum inflammation; follow your dental team's guidance rather than using them indefinitely by default.

Interdental Tools: Finding What Fits

Because the contours around implants vary, interdental cleaning is where personal instruction matters most. Common tools include:

  • Interdental brushes: small cone- or cylinder-shaped brushes that clean the spaces beside and beneath implant crowns. Choose sizes that fill the space without forcing; plastic-coated wires are often suggested around implants.
  • Implant-specific floss and threaders: thicker, spongy flosses (and floss threaders for bridges) designed to sweep wide spaces and slide beneath connected restorations.
  • Water flossers: pulsed-water irrigators that flush debris from around implants and under full-arch prostheses; studies support them as a useful supplement, particularly where dexterity or access is limited.
  • Single-tuft brushes: a small pointed brush head that traces the gumline of an individual implant crown, useful for back-of-the-mouth implants and along full-arch frameworks.

No single tool suits every mouth. Ask your hygienist to demonstrate, in your own mouth, which tools reach your implant's contours; a short demonstration is often more useful than any product purchase.

Professional Maintenance Visits

Home care controls daily plaque; professional maintenance catches what home care misses and monitors what you cannot see. A typical implant maintenance program includes:

  • Regular recall visits, commonly every three to six months, tailored to your gum health history and risk factors.
  • Peri-implant assessment: the tissues around each implant are checked for redness, bleeding on gentle probing, and changes in probing depth, which are early indicators measurable before you would notice anything.
  • Professional cleaning with implant-safe instruments: hygienists use instruments and polishing approaches selected to remove deposits without scratching implant components.
  • Periodic X-rays: taken at intervals to confirm the bone level around the implant remains stable over the years.
  • Bite and component checks: the crown, screws, and any denture attachments are checked for looseness or wear, and your bite is assessed for overload.

Peri-Implant Mucositis vs Peri-Implantitis

The two stages of peri-implant disease are worth understanding, because the difference between them is the difference between a reversible condition and a serious one:

  • Peri-implant mucositis is inflammation confined to the soft tissue around an implant, the counterpart of gingivitis around a natural tooth. The gum may look red or puffy and bleed when brushed, but the bone is not yet affected. At this stage the condition is reversible: professional cleaning plus improved home care typically restores healthy tissue.
  • Peri-implantitis is the progression of that inflammation into the supporting bone, the counterpart of periodontitis. Bone around the implant is progressively lost, pockets deepen, and if unchecked the implant loosens and can fail. Treatment is more involved (deep cleaning, antimicrobials, and sometimes surgical procedures to access and clean the implant surface), and lost bone does not reliably grow back.

Because mucositis is usually silent apart from mild bleeding, and because the transition to peri-implantitis can happen without pain, routine monitoring, not the absence of symptoms, is what protects an implant.

Warning Signs to Take Seriously

Contact your dental office promptly if you notice any of the following around an implant:

  • Bleeding when brushing or flossing around the implant
  • Red, swollen, or tender gum tissue at the implant site
  • A bad taste or persistent bad breath localized to the area
  • Pus or discharge from the gumline around the implant
  • The gum pulling away or the metal collar of the implant becoming visible
  • A feeling that the crown or the implant itself is loose, clicks, or rotates
  • Discomfort or pain when chewing on the implant

A loose crown is often a minor component issue fixed quickly, but it should never be ignored, because looseness changes how forces reach the implant and can let bacteria into the connection.

Habits That Threaten Implants

  • Smoking and vaping: nicotine reduces blood flow to the gums, masks bleeding (hiding early warning signs), and is consistently associated in the literature with higher rates of peri-implantitis and implant loss. Cutting down helps; quitting helps more, at any stage.
  • Bruxism (grinding and clenching): implants lack the shock-absorbing ligament of natural teeth, so grinding transmits full force to the crown, screws, and bone. Unmanaged bruxism contributes to chipped restorations, loosened screws, and bone stress.   • Using teeth as tools: opening packaging, biting nails, chewing ice or pens. These point loads are hard on natural teeth and harder on implant crowns.
  • Skipping maintenance: the single most controllable risk factor. Implants placed and then unmonitored for years are where peri-implantitis is most often found late.
  • Uncontrolled health conditions: poorly controlled diabetes and untreated gum disease elsewhere in the mouth both raise the risk around implants, a reminder that implant health is connected to overall health.
