MegaGen Advanced Intermezzo
Advanced Intermezzo is MegaGen's answer to the narrowest gaps in the mouth, a one-piece implant published in diameters down to 2.0 mm, aimed at thin ridges and small front teeth where a standard implant simply will not fit.

What it is for
MegaGen positions Advanced Intermezzo as a strong solution for narrow ridges and anterior teeth, and as a choice for immediate placement in the anterior region and in narrow spaces.
Two situations create that need. Bone shrinks in width as well as height after a tooth is lost, so a ridge can become too thin for a standard implant even where height is fine. And lower front teeth are naturally small and close together, a standard-diameter implant may not fit between the roots of its neighbours without crowding them. At 2.0 mm at its narrowest, this implant is built for exactly those constraints.

MegaGen also publishes a post height of 7.5 mm, a post angle of 6° a thread pitch of 0.7 mm, and a maximum insertion torque of 60 Ncm. The catalogue's dimension drawing lists a thread length one millimetre shorter than each figure above; the lengths given here are the fixture lengths tied to MegaGen's own ordering codes.
The one-piece question, and MegaGen's answer to part of it
One-piece implants have a recurring practical annoyance: the fixture can jam onto the driver used to insert it. MegaGen states its double connection design drastically reduces this “stuck phenomenon” between fixture and connector, describing it as a chronic problem with existing one-piece products, and notes that a stuck fixture can be freed with a 1.2 hex hand driver.
The system also carries a guide slot for identification and correction during placement, and a check line that lets the dentist confirm how deep the fixture is sitting and whether the prosthetic connection is fully seated. Placing the hex low on the post means the implant can still be engaged and removed even if the upper part of the post has been trimmed away, which, on a one-piece implant that often needs shaping, is a genuinely useful piece of design.
The limitation that does not go away
None of the above changes the fundamental constraint. The post emerging through your gum is machined in line with the implant body, so its angle is fixed the moment the implant is placed and cannot be corrected afterwards. The crown is generally cemented rather than screwed on, which means removing it later usually means destroying and remaking it.
MegaGen's own clinical cases in this catalogue show provisional restorations carried for around three months before definitive prostheses at four and a half to five months, so this is not a fast-track treatment, and the provisional stage matters for shaping the gum.
Design details worth understanding
Narrow neck. MegaGen states the neck is narrower than the widest part of the thread, which allows the implant's depth to be adjusted to the clinical situation rather than fixed by the neck's width.
Concave cuff. The collar is concave, which MegaGen describes as helping the papilla, the small triangle of gum between teeth. At a front tooth, whether that papilla fills in properly is a large part of whether the result looks natural.
KnifeThread and cutting edge. The same knife-edge thread philosophy as AnyRidge, with a cutting edge providing a bone-tapping function for smoother insertion.
Surface and material. MegaGen specifies an S-L-A surface, sandblasted, large grit, acid-etched, the industry-standard roughening method, and states it is homogeneous across the implant. The material is pure titanium grade 4, cold worked, which MegaGen relates to strength and torsion resistance. Note this differs from the Xpeed surface used elsewhere in MegaGen's range.
